**Note:  I created this tracker information printed on plain paper for myself a couple of years ago but fancied a 6" X 9" journal for easy record keeping, so this is the notebook style. As a survivor of migraines (over 45 years), it was essential to record my daily migraine headaches, especially for visits to my neurologist monthly.  I've purchased other trackers in the past; however, filling out too many details wasn't suitable for me.  I just required a quick and effortless record.
